Throughout the playable world of Diablo 4, players can stumble upon Rare boss spawns in the open world, offering players the chance to kill some enemies, and get some rather interesting, yet guaranteed loot rewards for their efforts. Each of these enemies grant some decent perks, fitting for a few different playstyles. So, to assist you with hunting the Diablo 4 Rare Spawns and Bosses, we’ve made a list, helping you find the rare spawn and get those unique rare items.
There are plenty of rare spawn bosses in Diablo 4. Some of them are part of a challenge, others just give great leveling loot.
Diablo 4 Rare Spawns, Bosses, and Rare Item Rewards¶
Here’s a map showing you where you can find all the Diablo 4 Extremely Rare Spawns, for your Trophy Hunter achievement, and your fancy special gear.
Below we will go through the rare spawns in each region.
Also, some of these creatures are part of a Trophy Hunter in-game challenge. So, you can use this list to help you through the challenge. Note that you only need 15 to get the Trophy Hunter achievement in Diablo 4. The 15 you need for the Trophy Hunter achievement are:

Note that the 15 for the achievement, and the other 5 they placed around the world drop some interesting weapons and items. 
Bear in mind these items can go Ancestral and Sacred, so, if one of these items is very good for your build, like the Rabies pants, or the poison dagger, then you can repeatedly farm them if you so wish, until something with actual BiS stats drops and you can safely replace them. Kripp, likely the reason why you are here, mentioned about Ancestral and Sacred rolls on these and then enchanting them, so these can serve as both leveling and end game gearing processes if you have a particular stat you want.
