Why Does the Holy Chalice Keep Appearing in Kyovashad?¶

To put it bluntly, the Holy Chalice spawn is a bug in Diablo 4. It doesn’t affect gameplay or progression in any way, so you don’t have anything to worry about - you can safely just ignore it.

Normally the Holy Chalice is a key item in the Malady of the Soul questline in Kyovashad, which includes the eponymous Malady of the Soul, Depths of Despair and Faith in Blood sidequests. It appears in all three of these side quests as an item you need to use at Sister Octavia’s request in order to assist in various exorcisms, which naturally ends with you having to fend off some hellspawn or another. Normally the Holy Chalice is removed from your inventory during the quest after it is used in the aforementioned exorcism ritual, but for some reason it’ll continue to spawn in Kyovashad during World Tier 4 (Torment difficulty). While we don’t currently know if this only happens if you’ve started the Malady of the Soul questline, if it’s in progress, or if it persists after you’ve completed it, picking up extraneous Holy Chalices doesn’t affect the progression of this side quest, and redundant copies will be removed when you complete the aforementioned side quests.
