There are many different side quests to be found in Metaphor: ReFantazio, which will be spread throughout the game. Pagan’s Dilemma will probably be one of the first ones you find, and requires you to help procure a Breath of Fresh Air for a Mustari Man’s daughter. This page will detail where to start Pagan’s Dilemma, as well as the location of the Breath of Fresh Air item.
Pagan’s Dilemma Location¶
As you progress through the main story, you will end up making the inn in Grand Trad your temporary base of operations. This will open up the time passing mechanic, as well as side quests, and this one will appear in Grand Trad. In fact, as soon as you leave the inn, it will be pretty much right in your face, as the NPC with it will be right there in the area just outside of the inn. Speak to the Mustari Man to hear his plight, as his daughter is sick and the only cure he knows of is the Breath of Fresh Air item at the church.

However, considering his race, the church won’t offer to sale him the item, which is why he asks you about it. Offer to purchase the item for him and the side quest will begin. Note that this side quest has a deadline of 6/16, so if you don’t finish it by then, it will be gone forever.
Breath of Fresh Air Location¶
The Breath of Fresh Air can be found at the St. Fermis Church in Grand Trad. Specifically, it’s on Sunlumeo Street, which is the first area of the city. If you open your map, you can fast travel to the location by selecting it from the list on the right. Once you arrive on Sunlumeo Street, head to the building in the northwest corner of the map, which is the church. Head inside and go straight to the altar at the front, where you’ll want to speak to the Sanctor.
He won’t turn you away and will sell you the Breath of Fresh Air, but at an increased cost, due to you being an Elda. It will cost you 500 Reeves, so make sure you have that amount on you. Purchase the item from the church, then return to the Mustari Man in Sunshade Row.

Pagan’s Dilemma Rewards¶
For completing the delivery of the Breath of Fresh Air item, you will receive a Rusted Accessory. Head to any shop to get it appraised, but the item you get seems to be random. It seems this one will be a basic accessory that increases one of your stats by three points. Additionally, you will get +4 Tolerance, which is one of the Royal Virtues.
