Should You Let Volo Give You a Lobotomy in Baldur's Gate 3?

After saving Volo from the Goblin Camp, you can tell him to hang around at your camp. Once you’ve spoken to him at your camp for the first time, he’ll tell you that he can do research into finding a cure for the Mindflayer’s tadpole in your head. This page details all you need to know about whether Volo has the medical expertise to cure you and whether you should let him perform a rather dangerous surgery on you!

Should You Let Volo Remove the Tadpole?

The next time you speak to Volo at your camp after he first arrives there, you can ask him if his research has found anything to help with the parasite problem. He will immediately escort you to a bed you can lie on. He suggests that he could extract the tadpole as it isn’t too deep in your skull.

All signs at this point suggest that you should absolutely not let this inexperienced man anywhere near your brain with a needle. However, we’re here to tell you to trust him as the outcome is actually pretty cool!

Firstly, you’ll need to agree to the procedure, and then throughout this pretty gruesome scene you’ll have to continue to wince through the pain and not let him stop…even when he brings out a literal ice pick to lobotomize your character with!

After he stabs into your brain with the lobotomy, you’ll be unsurprised to discover that he cannot remove the tadpole afterall. And, what’s worse is he’s taken your eye too! But don’t worry, he will pull out a fancy prosthetic eye from his pocket and tell you it’s way better than your last eyeball!

Once your eye has stopped bleeding (it only lasts a couple of seconds but whilst it’s bleeding you have disadvantage on perception checks), you’ll be able to head into your character info and see that you can now see "Invisibility" iconInvisibility. This means you can see Invisible creatures within 9m and creatures must succeed a Dexterity saving throw or lose their invisibility! So whilst Volo did absolutely perform a botched ice-pick lobotomy, at least you got a cool eye in exchange!
