Andariel’s Visage grants its wearer an interesting collection of boons, including +2 Skills, +20% Increased Attack Speed, 8 - 10% Life Leech and +25 - 30 Strength, making it obviously geared towards physical damage dealers. Keeping with lore, it also gives massive boosts to poison resistance (Poison Resist +70% and +10% Max Poison Resist) while incurring a penalty to fire resistance (Fire Resist -30%). This vulnerability can be offset by socketing the helmet via [Larzuk] and inserting a Ral rune, or better yet, a jewel which offsets the fire resistance penalty. Even if you can’t find a use for it on one of your characters, it’s an excellent endgame mercenary helmet due to its Attack Speed and Life Leech.

Andariel’s Visage has a high iLv (85) and its base item (Demonhead) it an elite-tier Mask. Fortunately, it’s just at the edge of items Hell Mephisto can drop, and while it’s fairly rare, if you do enough runs you should see it eventually.
