This trophy is missable. You need the skill “Heart of the Jaguar” which you can purchase from the red skill tree. Collect plants to craft Endurance herbal mixtures - these reduce damage taken from melee and ranged attacks.
While it’s active, perform melee takedowns on enemies, either with the axe or with stealth. Weapon kills do not count.
