This quest can be obtained from Jezhek of Holohlavy, who can only be found during the Polite Armed Man encounter when fast travelling.
Description
I met an old knight, Jezhek of Holohlavy, who had lost his horse and armour in a duel. I promised I’d try to get them back for him.

This side quest can only be triggered during a fast-travel event. Fast travelling between 
Devil’s Den and 
Miskowitz will reliably take you through the path on which it spawns, but you may need to repeat the trip multiple times in order for the quest to trigger.
