At the base of the tower (the room with giant scorpions) you’ll find a doorway obscured by the broken walls and rubble. The doorway is to the right of the scorpion room if you’re entering it, left if exiting.
This will lead to a room full of Manflies. Defeat them and head into the next room where you’ll find a fragment and a 
Miquella’s Cross. Exit the room the other side and turn left to find an altar containing the Talisman.
