After visiting the Hunter’s Dream, Iosefka will take over the room you began in and lock the doors. If you talk to the door she will give you a Iosefka’s Blood Vial , provided you do not have one already, up until you defeat Father Gascoigne.
After reaching Cathedral Ward:¶
Return to Iosefka and she will offer to harbor people, with a reward for each one that you send her way.

Before defeating Rom, the Vacuous Spider:¶
Once you reach the Forbidden Woods you can enter Iosefka’s Clinic, where she can be found in a room upstairs. She will ask you to leave and if you continue forward she will become hostile, but if you leave and continue to send her victims after learning of her true intentions she’ll reward you with Blue Elixirs x3 for each.
After defeating Rom, the Vacuous Spider:¶
If you leave, or never visit, and return after defeating Rom, the Vacuous Spider you can safely head upstairs to easily kill her on a table.
Drops:
- 3,200 Blood Echoes
- Oedon Writhe Rune (Before defeating Rom, the Vacuous Spider)
- One Third of Umbillical Cord (After defeating Rom, the Vacuous Spider)
