How to Obtain
After obtaining the 
Medicine Buddha Gourd, head to the Rakshasa Palace Shrine in Furnace Valley. From the Shrine, head over the nearby bridge and take the path on the left, leading to a room with an old man you can talk to. Exhaust his dialogue, then interact with the fire in the center of the room to upgrade the gourd to its highest level.
